Yet 15.6% of the American population is still uninsured. (services, april,2005) For a country that has the highest expenditure rates on healthcare, it is quite shocking that the public gets to face the crisis of high rising prices for healthcare. The two major problems faced by the healthcare system of USA include rapid increase in the expenditures made on health and disabilities and high costs for administration of health care issues which account for at least 26% of the total money in healthcare. The reasons for the rising c osts are many including the dependence on the high costs of new medical technologies, drugs and tests, the rising population of the elderly, the rising frequencies of road traffic accidence and criminal activities requiring medical attention, intensive work hours and rising pays of health care professionals and doctors, high awareness of medical problems requiring immediate and most of the times unnecessary medical attention, untoward issues like fraud and issues causing waste of administrative money, the internal situation and inflation rates of the health care system and many more reasons. TYPES OF MEDICAL COVERAGE Broadly speaking, health insurance can be of two types. First is the group health insurance in which an employee and his immediate family are covered by an insurance company that is either partially or mostly paid by the employer. This type of insurance comprises as high as 90% of the total insurance issued by the insurance companies. (DeNavas-Walt, 2008) Sometimes this type of insurance may not provide the necessary protection that an individual requires; in that case an additional aid with the help of individual insurance can be purchased. Second type is the individual insurance plan. This type of plan, as the name suggests, is individually tailored to anyone’s needs. This type of insurance is usually acquired when the group insurance is not enough or requires certain addition. In that case one can choose their own preference of insurance company and select as much coverage as they require. This means an extensive browsing of the options available in the market as the mark ups and premiums on different plans by different companies vary to a great extent. The group and individual insurance plans provide different types of coverage. SICK Score Study Research Sarah Mohammad Iqbal Chagani This project is pilot study of the clinical student’s preceptor ERC approval research named as â€Å"SICK score study† Introduction to the problem and topic Triaging pediatric patients is a difficult and challenging task as under-triaging could lead to sentinel events and medical errors. Since pediatric patients are more susceptible and less expressive than adult; they deteriorate quickly and many of their signs and symptoms could go either unnoticed or non-prioritized. The increased influx of pediatric patients in ED and changing trends of trained staffing could result in patients being under-triaged or missed resulting in misallocation of limited resources of ED. Numerous scores have been initiated to calculate the severity of illness in children, but all scores have either been introduced in ICU or critical care units. Since Emergency Department serves as the first door to the hospital, it should also have a scoring system so that patients could be sorted accordingly and care is not compromised. WHO has introduced ETAT guidelines for pediatric triage in emergency department but it could only be implemented after proper training and education of staff. A sign of Inflammation in sick children (SICK) score, created by Thompson, has been implemented in ED and is calculated using baseline vital signs and 4 important assessment questions. Since easy to calculate it can be implemented in ED without any specific training as vital signs are taken at the triage and the remaining parameters are verbally assessed but not documented. With this project, if significant sick score could be introduced, children triage could be made efficient and effective. Assessment phase Measurement of vital signs is a routine practice in all triage scales utilized in different hospitals for children visiting emergency department. On previous occasions, it has been seen that a lot of under triaging and over triaging has occurred resulting in missed priority levels of children and patients either being missed or not diagnosed appropriately. On two occasions, oncology pediatric patients were under-triaged resulting in sentinel events of mortality of those children in ED. It is not only important to monitor vital signs at triage but triage staff should be able to calculate severity score so that appropriate allocation of patients could take place. Currently, this hospital utilizes ESI priority system to allocate patients in ED, however, it would be interesting to observe the relationship of SICK score with the outcome of patients and compare it to the Priority levels. Vital signs play a very important role in triaging and disposition decision making of pediatric patients. Respiratory rate, color, nasal flaring all are indicators of pediatric clinical condition, however, pulse oximetry is a non- invasive technique of ruling out the severity of illness in pediatric patients. In a study, pulse oximetry measures were taken to identify the change in treatment plan and decision making of physicians. For 305 patients with Sao2 values less than 95%, the clinicians ordered 81 additional diagnostic tests for 62 patients (20%) and 39 additional treatments for 33 children (11%), and changed or added diagnoses for 25 children (8.2%) (Mower, Sachs, Niscklin, Baraff, 1997). Of this same group, clinicians admitted 50 (68%) children after rechecking the oxygen saturation, whereas 23 children were discharged without having their pulse oximetry rechecked (Mower, Sachs, Niscklin, Baraff, 1997). Furthermore, mortality levels could also be predicted with the vital signs and conscious levels of pediatric patients. Among 1133 patients, abnormality in heart rate and respiratory rates had no effect on the mortality of patients but mortality increased as the age of pediatric patients decreased with Odds ratio being 5.2, 15.4, 42.6, 57.0 and 98.3 (Kumar, Thomas, Singhal, Puliyel Sreenivas, 2003).Unlike heart rate and respiratory rate, abnormal blood pressure, oxygen saturation and conscious levels were found to have a significant (p-value Abnormal vital signs have also been found to have a significant relationship with serious and life threatening infections in pediatric patients. Children with serious infection as compared to minor infection had significantly higher temperature >39C (p,0.001), tachycardia (p,0.001), tachypnea (p = 0.002), oxygen saturations (94% (p2 seconds (p39uC (p = 0.004), tachycardia (p2 seconds (p = 0.001) when compared with minor infection (Thompson, Coad, Harnden, Mayon, Perera Mant, 2009). The Unit Receptionists were informed by Manager and HN regarding the forms and reinforcement was provided by clinical student. E-mails were sent to the rotating PGME pediatric residents and pediatric ED faculty members regarding a brief of the study and requesting for filling of forms. A box was introduced in the pediatric area near the working desk of doctors for collection of data forms. Since the study was already funded by an international agency, therefore, the photocopies of the forms were done by the preceptor as per the need identified by the clinical student. Implementation phase The collected data was entered in SPSS software and codes were given to the categorical data. The sick scores were calculated using software provided by the preceptor. The total forms collected during the three week time was 185 forms but only 100 were included as the remaining had missing information (Diagram 1) The descriptive statistics of the variables revealed mean SICK score of 1.89 and a standard deviation of 0.98with the range between 0.00 – 5.10. Other variable statistics are defined in Table 1. Table 1 As displayed in Table 2, discharge patients accounted for 61% of the population and admission were 39%. It could also be noted that 38% patients lie in the age range 1-5 years and minimum percentage of 6% could be seen in the age range Table 2 On calculating the odds ratio (Table 3), it was found that SICK score had higher odds of 2.123 of patients being admitted to the hospital. Along with this, capillary refill also has higher odds of 2.46 to predict the admission outcome of patient. Table 3 Surprisingly, triage category and conscious levels were found to have no effect on the odds of outcome – indicating either discrepancies in the assessment tool of the study or the training of the triage staff. Consciousness plays a very important role in decision making of the patient but the results of this study are directing towards consciousness level having no effect on the odds of the outcome – which could be an error of the assessment tool. Though the model was 71.7% accurate in predicting the outcome of patient but the results are also implying towards comparing the assessment tool used in the Indian SICK score study and the tool used in this project. Evaluation This project has identified the importance of calculating SICK score in identifying the level of severity of illness in pediatric patients for appropriate triaging and allocation in emergency department. It also indicates that other than the baseline vital signs capillary refill should also be initiated at the triage as on an individual level it has higher odds of determining the outcome of patients. But the results also suggest few errors either in the tool formulated or the triage staff training which needs to corrected before the final study is initiated. Missing values was also a major problem as a lot of forms and patient data had to be removed as the forms were not being filled completely. Recommendation would include comparing the sick score with another simpler scoring system like TOPR and also identifying the relationship between SICK score and the mortality of pediatric patients. The major constraint that Golding puts on the boys is the personality clash between Jack and Ralph. From the beginning, when Ralph is elected leader, Jack hates Ralph, and towards the end of the book, the feeling becomes mutual. Without Jack and Ralph’s problems, life would have been easy, and the ‘darkness of man’s heart’ would not have been conveyed to the reader. Jack shows ‘the darkness’ and if he and Ralph had just been friends, there would never have been an opportunity for Jack to show this darkness which lurked beneath the surface. Golding also uses the dead pilot conveniently against the boys - the way in which he is caught in the trees just in the right position to be caught by the wind and look like the beast and the way the wind picks up after Simon has let him down from the trees and carries him out to sea, so that the other boys cannot see that it wasn’t a beast. The author uses the boy’s fear against them, and although this could possibly happen in the situation, Golding uses it as a weapon against them, their morale and their companionship. I think that the boys split up and go to Jack because of the fear - he can kill the beast, he can get them meat, and if they ever get upset, he can start a dance and all will be fine.

Introduction Solid Waste Management, the collecting, treating, and disposing of solid material that is discarded because it has served its purpose or is no longer useful. Improper disposal of municipal solid waste can create unsanitary conditions, and these conditions in turn can lead to pollution of the environment and to outbreaks of vector-borne disease—that is, diseases spread by rodents and insects. The tasks of solid-waste management present complex technical challenges. They also pose a wide variety of administrative, economic, and social problems that must be managed and solved.the unwanted or useless solid materials generated from combinedresidential, industrial and commercial activities in a given area. It may be categorizedaccording to its origin (domestic, industrial, commercial, construction or institutional);according to its contents (organic material, glass, metal, plastic paper etc); or according to hazard potential (toxic, non-toxin, flammable, radioactive, infectious etc) .Management of solid waste reduces or eliminates adverse impacts on the environmentand human health and supports economic development and improved quality of life. Furthermore, the distribution of waste generation in the different regions of a countryis indicative of its degree of urbanization. In cities, where standard of living is higher, there isusually a higher waste output compared to rural areas. This is reflective of the case of thePhilippines where its capital and largest urban centre, Metro Manila, generates a lmost a quarter ofthe country’s total waste generation (WB, 2001).The issue of solid waste in Metro Manila gained international attention in the 1980s throughthe Smokey Mountain dumpsite, which became representative of poverty in the Philippines(Gonzales, 2002). It used to be the country’s largest dump and is also one of the largest slums in the world (Yashwant, 2002). The government was concerned about the negative image that thisgave the country that it was eventually closed in 1995.The matter of solid waste and its management had, once again, catapulted itself into thelimelight in 2000, during which a number of issues plagued the solid waste management (SWM)sector. First came the ‘Payatas Tragedy’ in July when a huge amount of garbage in Payatas, the largest active open dumpsite in the Philippines, was loosened by heavy rains and caved in on thecommunity of scavengers living around it. Next came the impending premature closure of the SanMateo Waste Disp osal Facility (SMWDF), which took in majority of Metro Manila’s solid waste atthat time, at the end of the year which aroused panic and concerns among the Local GovernmentUnits (LGUs) and the Metro Manila Development Authority (MMDA), the bodies responsible forSWM in Metro Manila. They scurried around, desperate for alternative dumpsites and solutions. They also rallied for the repeal of the Clean Air Act (Republic Act 8749), passed the year before,under which incineration of solid wastes was banned.During this time, the Ecological Solid Waste Management Act (RA 9003), a landmark lawwhich addresses SWM issues was in the final stages of formulation. It was finally approved inJanuary of 2001 after delays due to the political perturbation at that time. The passing into law ofRA 9003 was met by much enthusiasm by political leaders and non-governmental organizations(NGOs) alike as it was the first law in the country that addressed SWM in a non-piecemeal manner.Despite the initial optimism after RA 9003’s approval, 2001 turned out to be the year of theMetro Manila garbage crisis. The LGUs and the MMDA provided neither alternative dumpsites norsolutions by the time SMWDF was shut. Garbage dumps were literally growing everywhere in themetropolis – roadsides, waterways, vacant lots, rivers and other public places. The problem got soout of hand that Metro Manila was declared as under a ‘state of emergency’ to obtain assistancefrom the national government (The Manila Times, 2002). The crisis also caught the attention of theWorld Bank, thus its focus on the growing problem of solid waste in the Philippines in the 2001Philippine Environment Monitor (WB, 2001).Things had momentarily quieted down with the opening of the Montalban Solid Waste Disposal Facility (MSWDF) in mid-2002. The issue of SWM, however, has not completely fadedinto the background as earlier events had inspired the activism of several NGOs. Unfortunately,though, it also escalated the growing not-in-my-backyard (NIMBY) problem, making the task ofsearching for locations for future solid waste disposal facilities for the LGUs and the MMDA abigger problem. These are just a few of the challenges that face the SWM sector. By looking at itas a system, its relationships and effects with the different elements in the sector can be identi fied.With this, improvements can be proposed and evaluated. 1. The personalities of the contenders Essay How significant were the personalities of the contenders to succeed Lenin in accounting for Stalin’s defeat of his opponents in the years 1924-29? Lenins death on the 21st January 1924 caused huge sadness across the country. After Lenins death, everyone was eagerly waiting to know who would be the next leader of Russia. In the years after Lenins death, there was no clear successor to his leadership. However, when Lenin was leader, Trotsky was always there for him, and he played a huge role in the Civil War, therefore everyone thought he would become Lenins successor. However, as well as Trotsky there were other significant contenders, such as Stalin, Bukharin, Zinoviev and Kamenev which created a huge power struggle. I believe that the personalities of the contenders were significant in this struggle. However, other factors such as ideological battles, removal of opponents by Stalin and the consolidated powerbases also had an effect. The personalities of all were all extremely different by a great margin. The most likely to win, Trotsky was intellectually gifted which greatly benefitted him. He was also a great military leader and had good organisational skills. Trotsky has a few strong disadvantages to his personality though. His late arrival to the Bolshevik party could be interpret Trotsky as an unloyal character while his arrogance due to his intelligence made him quite a few different enemies in the Bolshevik party. Trotsky’s arrogance led to his defeat as he vastly underestimated Stalin and the threat he posed, while also decreasing his inner party support. At the time Trotsky would be clear favourite to win but his poor characteristics let him down. The next contender and winner of the battle for power was Joseph Stalin. An underdog and one of the least likely looking people to win, Stalin played his cards well and to his advantage. Being a Bolshevik from the beginning supported the fact that Stalin was a loyal Bolshevik which was a strength of his personality. Stalin being a cunning character was another strength of his and was a contributing factor to the defeat of his enemies as he was easily able to manipulate and outplay them. His ruthlessness and his ability to easily gain support due to his position and authority to promote them also benefitted greatly to the defeat of his enemies. He had a major weakness and thorn in his side though and that was Lenin’s opinion of him. The ability to easily manipulate Stalin was also a major disadvantage for him. While Stalin didn’t look it, he had just as big  of a chance as Trotsky due to his nature. Another contender in the race for leadership was the â€Å"Golden boy† known as Bukharin. Bukharin was possessed one of the best personalities of the party and his traits and strengths such as his incorruptibility and honest and well natured manner were good aspects about him. His â€Å"Golden boy† title was also helpful as it represented how liked he was by Lenin before his death. His intellectuality also benefitted him greatly and he was often referred to as the brains of the Bolsheviks. Bukharin’s main weakness though was that he ha d no ambition and he was too young. These factors led to a small support for Bukharin while his right-wing views also cut out some votes for him. Another contender, Zinoviev possessed good strengths such as being a good speaker and being good friends with Lenin. These would essentially give Zinoviev a right to the leadership of the party. He was involved little in the civil war though and his dislikeable and vain nature did little to help him gain the support he needed which was why he teamed up with the last contender Kamenev. Kamenev was also a friend of Lenin but compared to Zinoviev was from a working class background which would have benefitted him greatly due to the say and respect he would have. Kamenev had little influence though and was too uninspirational greatly denting his chances Therefore, what we can infer from the personalities and strengths of each other contenders was that Stalin cleverly manipulated and took advantage of each contenders weaknesses which led to him becoming leader. With Zinoviev and Kamenev in decline due to the influence they had in the defeat of Trotsky, Stalin easily defeated them. When that had happened Stalin quickly turned against Bukharin and with his growing inf luence Stalin removed him from the race for leadership. At the end of everything, Stalin had removed all opponents and this was a big factor in determining Stalin’s victory due to the fact that it had removed all of the people in the way of Stalin becoming leader of the Bolsheviks. Another way Stalin took power was by increasing his influence by never actually taking a side. While everyone was on either a left-wing or right-wing stance, Stalin remained neutral as to not damage his influence with members of either side. This way Stalin would face less opposition due to him not going against a large opposition of the party. After being outrageously worn out, sweating away our lives at the factory, my stomach screams of hunger. Like a pack of starved lions being released and set toward their favorite prey, I feel the hunger boiling inside of me. I rush toward the already-forming line for nutrition. I reach the front of the line, grabbing a single slice of bread- the only dinner we are allowed. As I chew the tiny slice, I can sense the dry sourdough bread scratching my throat, like sand paper to wood. The pain that overtakes my body is unbearable, yet the thought of my beating heart that still circulates brings me joy; after all, this could be my last day alive. Standing with my family, we talk of the old days; the days when we were not starving for air or food, the days back in Hellenthal, Germany, â€Å"I miss your cooking, Momma. After finding her, the SS guards scream for order and silence. We jump from our places when we hear the barking demands, afraid of the blows that would come from their mallets if they find need to yell again. We both run out together to find mom and dad. Once we found the other remaining members of our family, we wait to find new directions from our camp leaders. â€Å"I wonder what’s going to happen. It always scares me when we have selections. † Lore confesses to us as we wait to be told where to stand. Franz's harsh words brought us unwanted fright throughout the next couple minutes. We are constantly scared for our well-beings, but on the days of selections, our worries triple. Who had I seen for the last time at the factory yesterday? Who from my past life, will be taken to the afterlife? These and many other thoughts rack my brains until I find the ultimate fear: Is this my turn? As the sun light glistens in the distant sky, my family and I decide to pray before the selection begins, but right as my father begins the starting words of the prayer, there is a loud commotion. Thunderous screaming came from Franz and the guards. The selection is beginning earlier then we had expected. We jump into our formation as quickly as possible. I keep Lore close to me, thinking it would help our chances. Mom is being pushed toward the adult women line, and dad is directed over with the men. I feel my heart ripping at the seams. Suddenly, I could feel Lore trembling in my arms, â€Å"Don't worry. Everything will be fine. † I assured her as a bend to whisper in her ear. I pray my words of comfort are correct. Before the selection starts, Franz explains that the ghettos are being completely demolished. He proclaimed that there will be a train departing at the end of selection, leading the selected to a new concentration camp: Auschwitz. We all wonder what horrors or dreams take place this camp. Is it anything like here? There are only two options, Auschwitz or death. Which is better, God? To begin selection, the general screams orders to the men, telling some of them to get on the train, and others to file into another line. That line is the one that no one wants to join, the line that leads to the walk of death. As the women are selected, the anxiousness in my chest begins to race. It is almost our turn. I can’t see what is happening to the adults, and I definitely can’t see my parents anymore. I have no clue where my parents have been sent. The general finishes with the women, and slowing his strides aim towards us, the poor defenseless children. It all happens so quick, everyone is selected at a rapid pace; Lore and I are still in working-shape, so we are sent on the train, lucky to live one more day. Many of our friends have been brought to the train also, which is a blessing. As we settle into our spots on the train, I feel a sharp tug on my ragged t-shirt, it was Lore, â€Å"Where are mommy and daddy? † Her simple and worried question sent me into a downward spiral. Lost in my happiness at surviving, I had forgotten to check on the whereabouts of my parents. Increasing my worry I quickly look around the train, for any sign of them. They aren't here. There aren't anywhere. There were only two lines created at the selection, one for Auschwitz and one for†¦. I suddenly realize where they had vanished to. I feel heart drop to the bottom of my soul: they are gone. Never to be seen again. Never to laugh with again. Never to see their smiling faces when I wake up in the morning. Never to run to them when in need. Never to provide protection. As I realize what the circumstances that surround me, I feel Lore's presence on my side as she waits for an answer. How do I explain to her that she will never see them again? How do I explain that she will never taste the appetizing dishes mom would set on her placemat at dinner? How do I explain she will never play baseball with dad in the yard again? How? But, under my depression, I can't help but feel a glimpse of relief, for my sister and I are still alive and together. As tears stream down my eyes, I realize Lore will always need me at her side. Yet I also know, she must now hear some of the hardest words, she will ever hear. I clear my throat to portray a sense of composure, to tell of the events that have happened within the last twenty minutes. As I begin the tale, the train's horn blows and I can see the sun setting in the distance out the window of the train. Just twenty-four hours ago, I was standing with my parents talking about how they will always be there to help us. Now I’m left in charge. As I build up the courage to take on this new-found responsibility, I feel the train slowly begin our ride, our newest journey, to Auschwitz.

I feel that the treatment of the Indians and Africans by the Europeans was completely unjustifiable. While the Indians and Africans were less technologically advanced and the Europeans were uneducated, in this particular field, nothing can compensate for the actions of the Europeans. As Europeans began to settle new lands they began their exploration of the foreign worlds. What they found was the opposite of what they expected. They found what they thought was a new breed of humans. In reality they were just Native Americans. These Indians were less technologically advanced than the Europeans. They also worshipped different and multiple gods and ate different foods. Europeans saw this as barbaric, so they treated them as barbarians. In the beginning Native Americans hadn’t the faintest idea of what the Europeans had in mind when they said trade. They figured that when the White Man came and showed all that hospitality they meant it. Of course, they didn’t, the Europeans captured the Indians to be used as slaves. They were also slaughtered and raped because of resistance to leave their land. If any Indians refused to leave their land they would be killed. The women were raped for sick and disgusting reasons. Europeans didn’t feel that the women, or men for that matter, were worth anything as humans so they were beat and raped without any thought about what they might be doing. As we watched the movie Roots, I noticed a part in the movie where they were on a ship and a man brings in a black woman, who was a slave. The man offered her to the ship’s captain and referred to her as a belly warmer. That got me to thinking what could be going on in that man’s head as he said that. When I heard that I was shocked that a man could treat someone like that. People have feelings and cannot be treated as objects. Maybe the Europeans didn’ t realize that these people were, in fact people, and that drove them to this awful conclusion that they could treat people this way. As Europeans settled their land and began to build houses, farms and plantations, they realized that they needed servants to assist them in their farming. So people would travel to Africa capture blacks and then sell them to merchants and plantation owners. They would then beat them and put them to long, grueling work. They would treat them as they did the Indians, and for much the same reason. They figured that since the blacks were black and appeared to be less advanced then they must be less significant. People of the next generation whose parents owned slaves and grew up thinking slaves were okay is understandable. I just don’t feel that anything could justify treating the slaves they way they did. they had absolutely no respect for them. They would savagely beat them to get them to work harder than humanly possible and they would rape the women. I don’t think that I will ever know how any one could do such a thing. I conclusion I strongly feel that the way Europeans treated people that were less technologically advanced is completely and utterly wrong. It is difficult to contemplate what was going on in their heads as they were capturing them, killing them and even raping them. I can not believe how they could think that the color of someone’s ones skin or religious beliefs could make them less human. I am glad I live in a country based on the belief that all men are created equal.
